The purpose of this assignment (from the EVOC 501 course at CSU San Bernardino) is to insure that ROP teachers working on meeting the requirements for a Preliminary Designated Subjects Teaching Credential, will get the information needed to meet the requirements for that credential in a timely manner before their credential expires!
The information and links on this page should be helpful to anyone who has a Preliminary Designated Subjects Teaching Credential, even if they do not plan on taking the EVOC courses offered by CSU, San Bernardino.
Any ROP teacher who has not yet met the requirements for clearing their Preliminary Designated Subjects Teaching Credential is advised to follow the links included below to find the answers needed to the questions posed and then to submit that report to their ROP supervisor/coordinator to confirm that what they have indicated in the report is indeed what they need to do.
PLEASE NOTE: Although this assignment was developed for ROP teachers, it should prove to be helpful to any teacher who has a Preliminary Designated Subjects Teaching Credential (either Vocational or Adult and either Part-Time or Full-Time).
PREPARATION OF THE CREDENTIAL STATUS REPORT
Follow the links associated with each question (or refer to local ROP resources) to find the answer for each question below. When the report is completed, discuss it with your ROP supervisor/coordinator to insure that your plan will enable you to meet credentialing requirements in a timely manner and certainly before your preliminary credential expires!
CREDENTIALING INFORMATION - Anyone who has applied for a Designated Subjects Teaching Credential (or is considering applying for one), is responsible for finding out what he/she must do relative to the processing of that credential. Please answer all of the following questions relative to the Designated Subjects Teaching Credential:
- What subject(s) should (or do) appear on your Preliminary Designated Subjects Teaching Credential (indicate if your credential is PART TIME or FULL TIME and ADULT or VOCATIONAL)?
The "subject" (what you are authorized to teach) should be indicated on the face of your Preliminary Credential. If you don't yet have a Preliminary Credential (or can't find it), select the name of a specific subject from the list of subjects posted by the California Commission on Teacher Credentialing (you will need to select either the "Designated Subjects Vocational Education," the "Designated Subjects Adult Education," or the "Designated Subjects Special Subjects" link to see the list of subjects). What specific Level 1 and Level 2 courses do you need to take to clear your credential? When and where do you expect to officially complete them?
List all of the required courses with the date you completed or expect to complete each course and the name of the institution where the courses will be (or have been) completed. Information about Level 1 and Level 2 online courses offered through CSU, San Bernardino, can be found at:

Also indicate how the following requirements have been, or will be, met:
CPR - required of all teachers

Any other requirements (such as TB test, etc., if applicable)
By what date will your Preliminary Designated Subjects Teaching Credential EXPIRE if the courses listed above are not completed?

What is the name, e-mail address and phone number of your credential analyst? And the name and address of the agency for which he/she works? Please also indicate the name and e-mail address of your ROP supervisor/coordinator.
Most ROPs have a staff person who can help teachers with the credentialing process and that is the best person to contact.
If you already have the Preliminary Credential, it is essential that you are in contact with the credential analyst (and/or your immediate supervisor) at the institution where you are teaching, in order to make sure that the requirements for clearing that credential are clearly understood!

REGISTRATION INFORMATION - Anyone planning on enrolling in EVOC courses at CSUSB or at any other institution that offers Designated Subjects credentialing courses, is responsible for finding out what he/she must do relative to meeting admission and registration requirements. Please answer all of the following questions about admission and registration:
- When will you apply for admission and officially enroll in the courses needed and what do you need to do to accomplish that?

- What specific courses do you plan to enroll in (indicate the day and time that each course will be offered)?
Indicate the course number, the day and the time for each course (such as "EVOC 501, Mondays 6 to 10 PM"). Information about courses offered at CSU, San Bernardino is posted at:

- Do you have any particular problems (challenges) relative to the admission or registration process?
